DESCRIPTION

Platform for the improvement and optimization of chronic patient management based on Artificial Intelligence techniques and non-invasive monitoring, modeling frailty states with the ability to predict and simulate their evolution, as well as the associated care pathways.

SmartChronic is oriented towards the development of tools to optimize the definition and deployment of integrated care pathways for chronic patients.

To this end, we are working on the development of a platform for the improvement and optimization of the management of chronic patients, based on Artificial Intelligence techniques and non-invasive monitoring, modeling the states of fragility with the capacity to predict and simulate their evolution, as well as the associated care itineraries.

Specifically, SmartChronic focuses on the development of a technological tool that applies predictive analysis techniques. This tool will help guide decision-making by professionals and managers, improve coordination within the healthcare system, as well as collaboration between patients, caregivers and service providers, with the aim of contributing to the efficiency and sustainability of the systems and improving the health conditions and quality of life of people.
